Over the past decade, Dana-Farber Cancer Institute has embraced a philosophy of patient- and familycentered
care. This approach has transformed the way the organization
delivers care. Patients and families are a driving force behind multiple ambulatory care initiatives, including a "fast track" program for pediatric patients with fever
and neutropenia, and a home hydration program. To illustrate the impact of this approach on patient safety, we
will describe the evolution of patient engagement and examine innovative programs in medication reconciliation
and patient-oriented teamwork training.
