D24/E24 Partnering with Patients to Create Safe Care: Pushing the Boundaries

December 11, 2008
9:30 AM to 10:45 AM

  • Maureen Connor, RN, MPH, Director of Risk Management, Dana-Farber Cancer Institute
  • Saul N. Weingart, MD, Vice President for Patient Safety, Dana-Farber Cancer Institute
  • Over the past decade, Dana-Farber Cancer Institute has embraced a philosophy of patient- and familycentered care. This approach has transformed the way the organization delivers care. Patients and families are a driving force behind multiple ambulatory care initiatives, including a "fast track" program for pediatric patients with fever and neutropenia, and a home hydration program. To illustrate the impact of this approach on patient safety, we will describe the evolution of patient engagement and examine innovative programs in medication reconciliation and patient-oriented teamwork training.

After this course you will be able to:
    • Identify opportunities to enhance patient participation in your organization
    • Pilot patient-oriented patient safety initiatives
